Appointment Requests

Please reach out with any questions. To ensure a timely intake and exit, we require prepayment for 1st time clients, and a fully completed and signed Health Intake and Policy Forms. (We will email these to you at the time of your booking) 

In order to ensure the safety and comfort of both therapist and client we ask that you have a 

-6ftX10ft unobstructed space that allows for privacy getting on and off the table. 

-Accessible Sink 

-Near outlet for Table Warmer (Optional)

-If you have animals, please secure them prior to arrival of therapist

We have a 24 hour Cancellation Policy. If you No-Show or Cancel within 24 hours of your appointment time, you will be charged 100% of Service Fee. 50% of this charge may be used toward a future appointment. We do require a card on file for booking.
